PROBLEM TO BE SOLVED: To provide an inspection image forming device for forming an inspection image that can surely inspect a flaw or the like in a hologram sheet with a hologram image drawn, using interference fringe concerning diffraction grating. SOLUTION: The inspection image forming device for forming the inspection image for inspecting the flaw of the hologram sheet with the hologram image drawn to be visible by the formation of interference fringe concerning diffraction grating and/or a lack or omission of the hologram image, has an irradiating means for irradiating the hologram sheet with light; a light receiving means for receiving reflected light from the hologram sheet; a dark color sheet arranged at parts other than the light receiving means; and an inspection image forming means for forming the inspection image corresponding to the part constituting the hologram image irradiated with light based on the received reflected light.
