A polyethylene sheet adhesive composition for lining a concrete water tank is provided to prevent the yellowing and white powder generation even after a long time and to prevent the elution of harmful chemical materials. A polyethylene sheet adhesive composition for lining a concrete water tank comprises 31-49 wt% of a diglycidyl ether bisphenol A type epoxy polymer; 9-12 wt% of a urethane polymer; 13-17 wt% of calcium carbonate; 8.924-10.375 wt% of activated carbon; 2-4 wt% of chlorinated polypropylene; 8-11.5 wt% of a ceramic powder; 0.075-0.125 wt% of an antibacterial agent; 6-8 wt% of an organic pigment; and 4-7 wt% of a plasticizer.
