PROBLEM TO BE SOLVED: To efficiently remove a form panel after placing concrete, for requiring to extract the form panel in the axial direction of a separator when removing the form panel, since the form panel cannot move in the vertical direction to the axial direction of the separator, when a through-hole for inserting the separator substantially coincides with an outer diameter of the separator, though the form panel must be removed for extracting this form panel, since the separator projects outside from the form panel, when removing the form panel. SOLUTION: The form panel 2 is easily attached-detached by a plurality of tools (such as a Phillips-head driver, and a hexagonal wrench), by arranging a pivot part 5 composed of the laterally predetermined length and having a male screw having the conical tip, a form panel interval holder 3 in a diametrically expanded shape of forming a nut shape peripherally arranged in a central part of the pivot part 5 and a fastening retainer 6 freely installable-removable by spanner sockets to the form fixture F, or by arranging a plurality of opening grooves 7 (such as a cross hole, and a hexagonal hole) on the tip of the pivot part 5.
