PROBLEM TO BE SOLVED: To increase flexural rigidity by using an inexpensive constitution; to improve concrete filling properties; and to enhance the fatigue durability of a reinforcing steel material. SOLUTION: A form 5 for a composite floor slab is manufactured by arranging a plurality of reinforcing steel materials, composed of section steel 2, on a steel bottom plate 13 in such a manner that they are mutually juxtaposed in a bridge-axis direction; the forms 5 are connected to each other at least in the bridge-axis direction; longitudinal reinforcements 17 orthogonal to at least upper flanges of the reinforcing steel materials are laid across the upper flanges of the reinforcing steel materials; and the reinforcing steel materials and the reinforcements 17 are constituted by being integrated with concrete.
