PROBLEM TO BE SOLVED: To provide an information-carrying sheet which can securely conceal personal information, and eliminates a covering sticker to be pasted after filling the information therein, and further enables leaving a copy of the filled information, being suitable especially when a general consumer sends his or her information to a business organization. SOLUTION: The information-carrying sheet is characterized by keeping a base layer including a secret information part, a carbon copy layer for copying information on the secret information part, and an information fill-in layer bonded by use of an adhesive.
