The instrument has a tongue spacer constituting a tubular sleeve that includes an end section (1) terminated by a spatula (4) and another end section (2) on which a handle (5) is mounted. The end sections have an angle ranging between 75 and 130 degrees. The spatula is orientably mounted on the end section (1). The spatula includes an end cover (3) fitted on a free end of the section of the sleeve in a removable manner. Inside of the sleeve communicates with holes of the spatula emerging at outside.
