PROBLEM TO BE SOLVED: To provide a hologram recording and playback apparatus that reduces error bits. SOLUTION: The hologram recording and playback apparatus 100 has a spatial light modulator 112 that modulates a light flux spatially and an objective lens 106 that collects object light and reference light for recording to a recording medium, in order to perform hologram recording of information. The spatial light modulator 112 is controlled by a control section that generates a two-dimensional digital pattern which is formed with n items of first subsets comprising sixteen elements and which corresponds to the information. A first subset includes four pairs of second subsets, each second subset includes four elements. Out of the four pairs of second subsets, in three pairs of second subsets, the control section makes one element out of the elements forming the second subsets a first state, makes the three elements a second state, and makes four elements forming the remaining one pair of second subsets out of the four pairs of second subsets, a second state.
