A high power laser handpiece is provided to prevent an edema and a burn due to laser heat by including a cooling device for processing high power laser heat and cooling an operated region of a patient. A high power laser handpiece includes an area adjusting device(30), a knob(20), and a cooling device(50). The area adjusting device changes an area of high power laser beams. The cooling device is in contact with a diseased part of a patient and cools a skin contact window. The area adjusting device includes a convex lens lump, a concave lens lump, and an area adjusting driving screw. The area of the laser beams is adjusted by changing a focal length through a change of a distance between the convex lens lump and the concave lens lump.
