PROBLEM TO BE SOLVED: To provide a form grinding wheel tool usable for a long period by suppressing the wear of a grinding wheel part. SOLUTION: This form grinding wheel tool 6 forms a grinding device for grinding, in a predetermined shape, the side surface of a workpiece held on a workpiece holding table, and is attached to the rotating spindle of a grinding unit. The form grinding wheel tool comprises an annular base having, at its center, an attaching hole for attaching it to the rotating spindle and having an outer peripheral surface so formed as to correspond to the side surface of the workpiece and the grinding wheel part 62 having abrasive grains electrodeposited and formed on the outer peripheral surface of the annular base.
