PROBLEM TO BE SOLVED: To easily determine that a flat type flexible substrate is released from an actuator. SOLUTION: An actuator unit 21 has a piezoelectric sheet 141, individual lands 136 corresponding to a number of individual electrodes arranged on the jointing face of the piezoelectric sheet 141, an inspection land 137, and a common electrode 134 arranged on the opposite face of the jointing face of the piezoelectric sheet 141. The inspection land 137 and the common electrode 134 are electrically connected with each other through internal wiring 134a. On the surface of a COF 50, output terminals 58 jointed with the individual lands 136 and an inspection terminal 59 jointed with the inspection land 137 are formed. A driver IC has an electric characteristic measuring part for measuring a resistance value between the inspection terminal 59 and the common electrode 134, and a determination part for determining whether or not the COF 50 is released from the jointing face of the actuator unit 21 from the measuring result of the electric characteristics measuring part.
