PROBLEM TO BE SOLVED: To provide a coated cutting tool (cemented carbide insert) particularly useful for wet type and dry type milling of low and medium alloyed steels having rough surfaces such as a cast surface, a forged surface, and a hot or cold rolled surface, or a preliminarily machined surface. SOLUTION: This insert is the coated cutting tool particularly useful for milling of low and medium alloyed steels with the rough surface or without the rough surface under the wet type or the dry type condition. The insert is characterized by: a WC-Co cemented carbide body with a low content of cubic carbides and a high W-alloyed binder phase; and coating including an inner layer of TiC<SB>X</SB>N<SB>Y</SB>O<SB>Z</SB>with columnar grains, an inner layer of<SB>k</SB>-Al<SB>2</SB>O<SB>3</SB>, and preferably an outermost layer of TiN.
