PROBLEM TO BE SOLVED: To provide a waveguide type optical modulator with output light monitor for precisely detecting radiation mode light radiated from an optical waveguide as monitoring output light even when using a thin plate as an dielectric substrate. SOLUTION: The waveguide type optical modulator with radiation mode light monitor comprises: an optical waveguide element provided with the dielectric substrate 1 and the optical waveguide 2 which is formed on the surface or the rear surface of the dielectric substrate and has at least one Mach-Zehnder type optical waveguide; and an optical detector 60 which receives a part of the radiation mode light 20 radiated from a multiplexing point of the Mach-Zehnder type optical waveguide. Further, the waveguide type optical modulator with radiation mode light monitor comprises: high refractive index means 50, 51 disposed near an output side optical waveguide extended from the multiplexing point of the Mach-Zehnder type optical waveguide and on at least one side of regions on the substrate which insert the output side optical waveguide therebetween; and the optical detector 60 disposed so as to directly detect the radiation mode light which has the normal direction component on substrate surface and is released to the outside of the substrate by using the high refractive index means.
