A read-only optical disk medium and a method of fabricating the same are provided to record additional information data as record data row of perforated marks so as to record unique additional information on each optical disk medium. A center hole(96) is formed on the center of a read-only optical disk(90). An information recorded area(1) is recorded with information by embossed pit array. The information recorded area includes a lead in area(91) recorded with management information, a main data area(92) recorded with content data, a lead out area(93), an additional write area(94) in which additional information is recorded via perforated marks, and a test write area(95) for test write before recording perforated marks.
