PROBLEM TO BE SOLVED: To provide an optical module in which an optical device can be mounted on a substrate in a simple and easy way by a passive alignment technique, a driving device can be disposed inside the optical module and a wiring route between the optical device and the driving device can be shortened, and to provide a manufacturing method therefor and an optical transmission/reception system using the same. SOLUTION: This optical module is so structured that: a needle-like projection 5 serving as an optical waveguide is formed on a substrate 2; a fixing groove 6 for holding and fixing an optical fiber 4 on a proximal end 53 side of the needle-like projection 5 is formed; an optical device 3 is mounted so that the light-receiving part or light-emitting part of the optical device 3 is positioned at a distal end 51 of the needle-like projection 5; the optical fiber 4 is held and fixed in the fixing groove; and the light from the light-emitting part of the optical element 3 is made incident on the distal end 51 of the needle-like projection 5 and propagated in the needle-like projection 5 and made incident on the optical fiber 4, or the light from the optical fiber 4 is propagated to the needle-like projection 5 and made incident on the light-receiving part of the optical element 3 from the distal end 51.
