A technique for the alloplasty of the giant postoperative ventral hernia with the diastasis of the rectus muscles of abdomen comprises the dissection of the postoperative cicatrix, the separation, the partial removal and the tailoring of the flaps of the hernial sac, the disengagement of the adhesions between the intestine, the greater omentum, and the edges of the hernial defect, the mobilization of the muscular-aponeurotic edges of the hernial defect from the subcutaneous base to the anterior aponeurotic walls of the oblique muscles of abdomen, the cut of the tendineous junctions of the aponeurotic sheaths of rectus and oblique muscles of abdomen, the reinforcement of the weak areas and the medial defect by the polypropylene mesh fixed to the muscular-aponeurotic edges of the hernial defect. For the reinforcement of the medial defect, the polypropylene mesh is placed between the flaps of the hernial sac and fixed to the muscular-aponeurotic tissues starting at the distance of 5-6 cm from the edges of the defect. |
