PROBLEM TO BE SOLVED: To enhance the strength of the support and positioning of a form plate, in a form structure in which a reinforcing-concrete placing space, including a reinforcing strip-shaped iron plate, is formed on the outside of an existing structure. SOLUTION: Fittings 1A and 1B with a U-shaped side surface, which are fitted into both the lateral sides of the strip-shaped iron plate P, are fixed to both the lateral sides of the strip-shaped iron plate P by means of a fastening bolt 10. On the side opposite to the side of the presence of the fastening bolt 10, a connecting separator S1 in the direction of crossing a section between both the lateral sides of the strip-shaped iron plate P is inserted through separator inserting portions 8 of both the fittings 1A and 1B, and fixed to the strip-shaped iron plate P by means of long nuts N1 and N2; separators S4 and S5 for extension are concentrically screwed and connected together by means of the long nuts N1 and N2; perpendicularly-directed separators S2 and S3 are screwed and connected to both the fittings 1A and 1B by means of a screw hole 7; and the form plates 13-15, which form a concrete placing space 16, are positioned by means of the respective separators S2-S5.
