PROBLEM TO BE SOLVED: To promote the smooth mutual understanding between a doctor and a patient by making effective use of an inquiry sheet and a consultation ticket. SOLUTION: The consultation ticket 1a has an entry of pieces 13a, 13b, 13c and 13d of information for the purpose of visiting a clinic on the back. A transparent consultation ticket case 1b has an opaque part 14 which can conceal a plurality of pieces of the pieces 13a, 13b, 13c and 13d of the information for the purpose of visiting the clinic.
