PROBLEM TO BE SOLVED: To improve reliability of products in both surfaces of a device and a storage medium, by stably and surely managing defect information and defect management information. SOLUTION: A system read-in region, a data read-in region, a data region, and a data read-out region are secured successively from the inner periphery in an information recording medium; land groups are formed alternately in these regions; the groups are amplitude-modulated; a physical address is allocated by this modulation information, recording information recorded on the land is adapted in a slice level detection system, in the data read-in region, the data region; and the data read-out region, recording information recorded on the land are recording forms adapted to high-density recording by a partial response maximum likelihood (PRML) system, and a defect management information region (DMA) for recording defect management information of at least the data read-in region and the data region is secured to the data read-in region.
