A three dimensional image display device and a manufacturing method thereof are provided to convert 2D(Dimensional) display into 3D or vice versa, thereby selectively implementing the 2D or 3D display. A display panel displays an image. A 3D image forming panel(100) is arranged on the display panel and converts 2D image from the display panel into 3D image. The 3D image forming panel includes plural lens forming guide members(120) formed on an insulating substrate. The first substrate(150) includes plural electrode layers formed on the lens forming guide members. The second substrate(180) is attached oppositely to the first substrate. A liquid crystal layer(190) is located between the first and second substrates. The size of the lens forming guide member is reduced gradually from the first substrate to the second substrate. The cross-section of the lens forming guide member is any one among triangle, semi-circle and trapezoid.
