PROBLEM TO BE SOLVED: To provide a device and a method for positioning and forming bone tunnels. SOLUTION: In one embodiment, a surgical drill guide apparatus 60 is provided. The apparatus can include a first guide member 12 having a longitudinal passage extending through the first guide member 12 for aiming a first guide pin 50a along a first path to form a first bone tunnel, and a second guide member 30 having a longitudinal passage extending through the second guide member 30 for aiming a second guide pin 50b along a second path to form a second bone tunnel.
