An information insertion apparatus, a printed matter inserted with information, an information insertion method and a computer readable recording medium recording an information insertion program are provided to change a feature amount of an image if a code is abnormally inserted, and insert a code based on a feature amount of a re-image, thereby enlarging a kind of images into which the code can be inserted. An information insertion apparatus(100) comprises an image input unit(101), a recording unit(102), an insertion code calculation processor(103), an encode processor(104), a decode check processor(105), an image output processor(106), and a gray change processor(107). The insertion code calculation processor extracts a feature amount from an image, and calculates a code of information inserted into the image based on the feature amount. The encode processor inserts the code, calculated by the insertion code calculation processor, into the image. The decode check processor determines whether the code is normally inserted into the image by the encode processor. If not so, the gray change processor changes the feature amount of the image.
