A three-dimensional image measuring apparatus using shadow Moire is provided to turn on/off a lighting unit selectively regardless of the shape of a measuring object to measure a three-dimensional image of the measuring object. A three-dimensional image measuring apparatus using shadow Moire comprises grating(110), a grating transfer unit(120), a beam splitter unit(130), an auxiliary lighting unit(140), an image forming unit(150), a plurality of lighting units(160) and a control unit(170). The grating is installed above a work stage(101) for transferring a measuring object(102) to a measuring position. The grating transfer unit is equipped with the grating and transfers the installed grating. The beam splitter unit is installed above the grating, irradiates first beam to the grating and transmits the reflective image of the measuring object through the grating. The auxiliary lighting unit is installed at a side of the beam splitter unit, and irradiates the first beam to the beam splitter unit. The image forming unit is installed above the beam splitter unit, and photographs the reflective image transmitting the beam splitter unit. The lighting units are installed to be respectively inclined around the image forming unit above the grating, and irradiates second beam to the grating. The control unit controls the grating transfer unit, the beam splitter unit, the auxiliary lighting unit, the image forming unit and the lighting units.
