PROBLEM TO BE SOLVED: To provide a document reader capable of preventing document reading defects caused by the oblique carrying of a document or the deviation of paper feeding timing with a simple configuration, and correctly performing document carrying which satisfies speeding-up, and to provide an image forming apparatus using the document reader. SOLUTION: The document reader 1A2 includes: a document tray 2B; and a document carrying device 1B, having a feeding roller 5A for introducing the document to the reader, a document carrying passage H1 for carrying the document, and carrying rollers R1-R3 for carrying the document, thereby automatically carrying the document to document reading positions S (S1, S2). In the document carrying device 1B, a first document detection sensor SW1 is arranged at the downstream side of the document carrying direction of the feeding roller 5A, and a second document detection sensor SW2 is arranged at the downstream side of the document carrying direction of the carrying roller R1.
