PROBLEM TO BE SOLVED: To generate an output image having a meaning of second information different from first information as an output image of position information or the first information. SOLUTION: A code pattern image generating device 10 includes a position code generator 20 which generates a position code characteristic of a position on an output form or the recording position of a document image recorded on the output form, a discrimination code generator 22 which generates a discrimination code for discriminating the output form or document information recorded on the output form, an additional image information generator 24 which generates a code pattern image by arranging the discrimination code in a section of a code block formed by arranging position codes in two dimensions, a pattern group selector which selects a pattern group corresponding to the control information, and a code pattern image generator 78 which generates at least one of the position code and discrimination code in a code pattern image by using a plurality of unit code patterns that the pattern group has.
