PROBLEM TO BE SOLVED: To provide an optical scanner in which a deflector is made compact and thin, and to provide an image forming apparatus. SOLUTION: The optical scanner has a light source which emits a light beam and a deflection means which deflects and repeatedly scans the light beam from the light source. A deflection disk 302 is used as the deflection means and the deflection disk 302 is composed of a diffraction part 101 which diffracts and deflects the light beam and a waveguide part 102 which guides the light beam diffracted to outside of the diffraction part 101 into the disk. Thus, the light beam which is made vertically incident to the disk face of the deflection disk 302 is deflected at the diffraction part 101 in the radial direction of the disk and guided to a scanning line via the waveguide part 102. A pattern for diffractive deflection is formed on the diffraction part 101 so that the deflection direction is arbitrarily changed in the rotation plane of the disk by the rotation of the deflection disk 302.
