PROBLEM TO BE SOLVED: To provide a novel light source module which can increase the output of the light sources using an LD bar and which is less likely to be influenced by the errors in positional relations among the LD bar/the module for the light source/an optical fiber; and to provide a novel light source device designed by combining the light source module and the LD bar. SOLUTION: The light source module couples the light from the LD bar 60 to one optical fiber 70, wherein the LD bar 60 is configured such that N pieces of the LD are integrally arranged in line so that each light emitting part can be continuous in a longitudinal direction on the LD bar. The module includes, as a luminous flux combining means, a coaxial collimating lens array 62; a beam-condensing coaxial lens 66; and a means 64 for luminous flux multilayering. The coaxial collimating lens array 62 is designed to array the collimating lenses, to which the lens diameters in a mutually orthogonal two directions were set, by the same pitch as the arrangement pitch of the LD with a direction having small diameter oriented in the array direction, based on a ratio of divergence angle in divergent luminous fluxes emitted from an individual LD in the LD bar. The coaxial collimating lens array 62 has the capability of changing the divergent luminous fluxes emitted from the individual LD in the LD bar into a parallel luminous flux that is mutually adjacent or closely contacted.
