<P>PROBLEM TO BE SOLVED: To realize thinning of a backlight unit by obtaining a sheet form light guide plate. <P>SOLUTION: The light guide plate 30 is formed by installing a lenticular lens 31 and a prism lens 32 at both faces of a sheet base material 30a. Then, a ridge line 31a of the lenticular lens 31 and the ridge line 32a of the prism lens 32 are nearly orthogonally installed to an installing axis Y. Moreover, the sheet base material 30A is pinched between a first press mold having a shape to form the lenticular lens 31 and a second press mold having a shape to form the prism lens 32, and by pressurizing the first press mold and the second press mold under heating, the sheet form light guide plate 30 is formed.
