<p><P>PROBLEM TO BE SOLVED: To provide a dog passport for a pet dog which allows a guardian to promptly obtain information on necessary/convenient items or the like as the identification of an owned dog/guardian, the identification of quarantine/vaccination, a health insurance card and various types of insurance cards or the like, and can be efficiently used as a portable type convenient notebook. <P>SOLUTION: A plurality of paper sheets are bound between two-folding type front cover sheet and rear cover sheet formed of a strong material like a true passport to form a booklet wherein papers can be developed in an open mode to be viewed. An essential/instruction/reference items or the like regarding the owned dog of the passport and the guardian (dog owner/seller or the like) are preliminarily printed on the rear face of the front cover sheet/the rear cover sheet or each paper, or printed/hand written or the like in an after-the-fact manner for impressing, furthermore photograph columns are provided to paste the photograph of the owned dog, thereby promptly obtaining information regarding the appearance/characters, clinical history, traveling/transition history, pedigree, quarantine/vaccination identification or the like of the owned dog as an individual as well as necessary/convenient items or the like of the guardian. <P>COPYRIGHT: (C)2008,JPO&INPIT</p> |
