PROBLEM TO BE SOLVED: To provide a long-length liquid jet apparatus at a low cost, a manufacturing method of the liquid jet head and a liquid jet apparatus equipped with the liquid jet head, an image forming apparatus and a long-length piezoelectric actuator at a low cost. SOLUTION: The liquid jet head includes a nozzle plate 3 having a plurality of nozzles 5 formed for jetting liquid therefrom, a plurality of discrete flow paths 6 formed in communication with the plurality of nozzles 3, a diaphragm member 6 having a flexible part 11A that forms at least one of the walls of each discrete flow path 6 and a plurality of piezoelectric element units 12 having a piezoelectric element member 13 fixed to a base member 14. Each piezoelectric element unit has a plurality of piezoelectric elements 12a divided by a plurality of slits 15 formed in the piezoelectric element member 13.
