<P>PROBLEM TO BE SOLVED: To provide a form making it possible to easily and surely specify a readable or copyable area for an image, and to provide an image reader and an image reading and forming apparatus which uses such a form as a document and can surely read or copy the image in the specified area, and an image forming apparatus capable of creating a document. <P>SOLUTION: Disclosed is the form S with an attached RFID tag T wherein area information showing the area where partial reading or copying of the image formed on the form is inhibited or permitted is electronically written.
