PROBLEM TO BE SOLVED: To provide an optical transmission module having a forward light monitor structure capable of detecting the output of a forward emitted light from a light emitting element without basing on the temperature or the reflection return light without bringing an enlarged apparatus, and accurately controls the output of the forward emitted light. SOLUTION: The module comprises a light emitting element 1, a lens 2 for passing a light emitted from the light emitting element 1, a board 3 for mounting the element 1 and the lens 2 on the upside, and a light receiving element 5 having a light emitter 5a in front of the element 1 in the traveling direction of the light emitted from the element 1; the receiving element 5 being disposed at a position lower than a region for passing a main emitted light to be incident on the lens without obstructing the transmission of the main emitted light, for receiving a light leaking from the element 1 to monitor the output of the emitted light from the output of the leaking light.
