A surgical system having a pneumatic manifold equipped with an integral air cylinder is provided to secure a reliable and simple structure by directly installing the air cylinder for operating a valve, in the pneumatic manifold. A surgical system having a pneumatic manifold equipped with an integral air cylinder is composed of: a surgical console having a cassette containing part; at least one pneumatic manifold positioned adjacently to the cassette containing part and provided with at least one bore; and the air cylinder expansibly kept in the bore and provided with a piston capable of being extended or contracted. The air cylinder has a port(26) for pneumatically connecting the air cylinder to the pneumatic manifold. A cassette(10) is installed in the cassette containing part and provided with an opening(18) through which the piston reciprocates.
