A means for recording information associated with vehicles involved in an accident comprising: a booklet 1 having a plurality of detachable sheets 4, each sheet having identical spaces for recording said information 3, each space preceded by text indicating the nature of the information to be recorded in each space, such that all recorded information is present on all sheets in the plurality of sheets wherein each of said detachable sheets is adapted for recording identical information 6 from each individual involved in said accident; whereupon conclusion of recordal of said information, a sheet of the plurality of sheets is detached and retained by each individual involved in said accident. |
