PROBLEM TO BE SOLVED: To provide a flat die form rolling machine for a worm in which the work is least moved, and the machining accuracy of a form-rolled surface is enhanced. SOLUTION: Columns 7, 8 with a pair of flat dies 2, 3 fixed thereto are turnably provided on column bases 11, 12 facing parallel to each other. The flat dies 2, 3 are inclined in a plane parallel to bottom faces of the flat dies. The lead angleβof the flat dies is corrected to the lead angleβ1 of the flat dies 2, 3 corresponding to the change of the work diameter during the form rolling, and the movement of a work 4 is controlled thereby. Thus, in the form rolling, as a tooth of a worm gear is raised from the biting part to the finishing part, the lead angle is corrected to the value corresponding to the change in the work diameter under the form rolling, and the rolling diameter is gradually increased in a corresponding manner.
