PROBLEM TO BE SOLVED: To prevent a form from generating curl by non-uniformly eliminating humidification when giving water to both sides of the form to perform humidification.

SOLUTION: A form humidification device symmetrically arranges humidifying means on both sides of a conveying passage conveying the form in a vertical direction, and performs humidification to the form conveyed on the conveying passage.
