<P>PROBLEM TO BE SOLVED: To provide a production method for producing an electronic pen form sheet individually identifiable one by one while associating it with a person or matter, and a specification system for easily specifying a user from information entered to the electronic pen form sheet based on the association. <P>SOLUTION: In the electronic pen form sheet production system 80, a user and a form are preliminarily set to each form sheet, and each form sheet is printed by a printer while arranging a different dot pattern for each form sheet. The dot pattern arranged on each form sheet and a pattern of each form sheet are printed on a form sheet of paper medium, whereby an electronic pen form sheet 62 is produced.
