PROBLEM TO BE SOLVED: To provide a delivery invoice which describes the information of an invoice, a specification slip, etc. on a singe sheet, and can be attached to a delivery article in a state that information covered by the specification slip etc. is concealed, and also, to secure a sufficient space for the specification slip, advertisement, etc. SOLUTION: One piece of sheet 1 with a two-layer structure is divided into a plurality of tiers of compartments 4, 5 and 6 by cutting lines 12 and 13. In addition, an invoice 9 is formed in the compartment 4 of the first tier which turns out to be a surface when folded, and at least one of the compartments 4, 5 and 6 belonging to the second and following tiers to be folded to the back of the invoice part 9 is used as the specification slip 10 and first and second attachment parts 7 and 8 are formed on both sides of the invoice 9.
