PROBLEM TO BE SOLVED: To inhibit optically recording information entry space from being recorded on the receiving form, which is required at an entry of optically reading information and not required at reading of an optically reading device by enabling it to be recognized visibly clearly at the entering period, and rendering its optically reading unnecessary during facsimile transmission. SOLUTION: Optically reading facsimile transmission form 1 is formed such that ancillary sheet 3 is separably bound on the lower part of a facsimile transmission sheet 2 having optically reading information entry spaces 4-11 printed with a light color that is dropped out with respect to the light source of an optically reading unit, and on the opposite surface of the auxillary sheet 3, auxiliary spaces are provided having a different color from the optically reading information entry spaces so as to distinctly recognize the optically reading information entry spaces 4-11 from the surface side of the facsimile transmission sheet 2. |
