The system has a medical shaft instrument e.g. cutting and gripping instrument, inserted into a hollow shaft (1). A set of cams (7) is arranged over an internal lateral surface of the hollow shaft in a distributed manner. One of the cams is arranged on an inner side of the hollow shaft for reducing the inner diameter of the hollow shaft, where the cams are arranged on a circular line. A recess (11) corresponding with the cam is formed in a shaft of the medical shaft instrument.
