PROBLEM TO BE SOLVED: To take a printed form on a paper discharge tray in an image forming apparatus and to use it as a recycled form. SOLUTION: The image forming apparatus comprises an image forming unit 124 which forms an image on a form, a paper feeding unit 125 which conveys the form from a paper feed tray and supplies it to the image forming unit 124, a recycled form feeding unit 126 which conveys the recycled form from a recycled form feeding tray and supplies it to the image forming unit 124, a paper discharge unit 127 which conveys an image-formed form having an image formed by the image forming unit 124 and supplies it to a paper discharge tray, a form taking-in unit 128 which takes the image-formed form from the paper discharge tray in an apparatus main body, a form deciding unit 129 for form decision making which decides whether the form taken in by the form taking-in unit 128 can be recycled, and a conveying means which conveys a form decided as a recyclable form by the form deciding unit 129 form decision making to the recycled paper feeding tray and also conveys a form decided as an unrecyclable form to a waste form tray.
