A method and a system for connecting a traffic call in a multimedia substitute ring back tone service are provided to offer a multimedia substitute ring back tone service through rapid call recognition by connecting a traffic call containing a multimedia ring back tone terminal flag and setting up an ISUP call when a mobile terminal attempts call origination. A system for connecting a traffic call in a multimedia substitute ring back tone service comprises a calling mobile terminal(110), a called mobile terminal(112), an originating MSC(140), a serving MSC(142), and an HLR(160). The calling mobile terminal(110) transmits an outgoing call containing a multimedia ring back tone terminal flag and makes a call request and a location registration request. The called mobile terminal(112) receives the call request from the calling mobile terminal(110). The originating MSC(140) receives the outgoing call, connects a traffic call containing the multimedia ring back tone terminal flag if a speech path is connected with the calling mobile terminal(110), and executes ISUP call setup. The serving MSC(142) executes ISUP call setup with the originating MSC(140), pages the called mobile terminal(112), and transmits a substitute ring back tone to the calling mobile terminal(110) if the called mobile terminal(112) is affiliated with the multimedia substitute ring back tone service. The HLR(160), when the calling mobile terminal(110) makes a location registration request, transmits a location registration result containing the multimedia ring back tone terminal flag to the originating MSC(140).
