<p><P>PROBLEM TO BE SOLVED: To provide a questionnaire information distribution server for achieving the recovery of reliable reply results of a questionnaire, and to provide a questionnaire information distribution method. Ž<P>SOLUTION: A questionnaire information distribution server 10, each time a request for obtaining questionnaire data is received from a communication terminal device 20, generates questionnaire data by rearranging the order of selection items of reply to a question, and transmits to the communication terminal device 20. For questionnaire data to which rearrangement of the order of the selection items is not allowed, the questionnaire information distribution server 10 changes selection items selected at default and transmits to the communication terminal device 20.
