PROBLEM TO BE SOLVED: To provide a flat plate-combined form panel which facilitates form panel erection work on a construction site before the placing of concrete, and which minimizes the occurrence of a warp. SOLUTION: In this flat plate-combined form panel 1, a flat-plate waterproof heat-insulating plate 2 is bonded throughout one side of flat-plate wooden plywood 3, and waterproof and moisture-proof treatment is applied to the other side (a surface which does not adhere to the flat-plate waterproof heat-insulating plate) 4 and butt end of the flat-plate wooden plywood by using a waterproof and moisture-proof means. The waterproof and moisture-proof means on the other side and butt end of the flat-plate wooden plywood employs coating or waterproofing by a waterproof coating, and firm adhesion by a moistureproof resin film.
