A form for a slab is provided to use for constructing various slab structures regardless of structural design and a construction method of a slab using the form for a slab is provided to shorten a period of construction and to reduce the cost of construction by installing and dismantling easily without any special equipment. The form(10) for a slab contains a rectangular plate(11) installed between a column and a column and provided with plural coupling holes to be coupled to a bracket of a form for a prefabricated column and plural deck plate coupling holes to couple with deck plates put on the lateral corners of the plate, and plural stiffening ribs(12) projected from the bottom of the plate in the length or width direction of the plate. The construction method of a slab using the form for a slab comprises the steps of: installing a form for a prefabricated column on a place for a column of a building; connecting the form for a slab to the pre-installed form for a prefabricated column; putting and coupling the end of a deck plate(300) on two lateral corners of the form for a slab; arranging reinforcing bars for a slab on the form for a slab and the deck plate; placing concrete on the form for a slab and the deck plate and curing the concrete surface; and after curing the concrete surface, dismantling the form for a slab and the deck plate.
