PROBLEM TO BE SOLVED: To provide a three-dimensional perforated sheet capable of controlling the spreading direction of liquid and being easily bent; and an efficient manufacturing method. SOLUTION: This three-dimensional perforated sheet 1 is formed with a plurality of rib parts 2, and openings 31 between the mutually adjoining rib parts 2 and 2, and the rib part 2 is alternatively formed with relatively high portions 21 and relatively low portions 22 in its longitudinal direction. This manufacturing method of the three-dimensional perforated sheet is characterized in forming the perforated sheet by introducing a raw material sheet between a first roll which alternately has perforation parts having multiple perforation pins in their circumferential face and gear-shaped engagement parts alternately threadedly engaged with projecting parts and recessed parts in their circumferential faces, in the axial length direction, and a second roll which alternately has spacer parts having circumferential faces mutually facing the perforation pins and gear-shaped engagement parts threadedly engaged with the engagement parts of the first roll, in the axial length direction.
