A first information recording layer located further from the light incidence side includes a read-only control data area (304a), a first test area (306a), and a second test area (315a). A second information layer located closer to the light incidence side includes a third test area (304b) located at a position opposite to the control data area (304a), and a fourth test area (315b) located at a position opposite to the second test area (315a).
