PROBLEM TO BE SOLVED: To obtain an image forming apparatus for forming a color image using toner of only three colors of cyan (C), magenta (M) and black (K) in which identification information can be added. SOLUTION: When a color image is formed, an image making unit of black (K) color system reproduces pixels 81-85 at the upper left corner of partial regions 71-75 representing apparatus series number "14402" as identification information in the block region 70 of 40 longitudinal pixels and 20 lateral pixels with black color.
