PROBLEM TO BE SOLVED: To provide a form with a self-addressed envelope which makes it possible to put the form in a sealed letter by only folding it up, without any labor of cutting off an application form part and an envelope part, and can give an expectation for a lot of replies. SOLUTION: The application form part, the envelope part and an information entering part are provided in one form and the application form part and the envelope part are so formed as not to require cutting off and so provided that they can be cut off from the information entering part. An adhesive coating part is provided in the lateral end portion of the envelope part opposite to the application form part. The adhesive coating part is provided also in the lateral end portion opposite to the lateral end portion wherein the adhesive coating part of the envelope part is provided, on the other side of the application form part positioned adjacently to the envelope part. The application form part is formed to be folded laterally and then longitudinally so that it is laid on the envelope part, and a seal piece is provided in the upper portion of the envelope part so that the application form part may enter this part without cutting off.
