PROBLEM TO BE SOLVED: To spread a resin to a square mold for a DOE lens up to four corners of the mold. SOLUTION: The UV curable resin 3 applied to a square pattern surface 2 is spread by a glass plate. The stepped part 5, which stores a UV curable resin 3 flowing out of a square pattern surface 2 having a concentric circular grooves 2a formed thereto when the UV curable resin 3 applied to the square pattern surface 2 is spread by a glass plate, is formed to the peripheral edge part of the pattern surface 2.
