PROBLEM TO BE SOLVED: To provide a clinical trial examination document creation device allowing easy creation of a proper clinical trial examination document.

SOLUTION: The device includes: a main screen output part 11 for outputting a screen having a node of a clinical trial concerned party, an arrow showing movement of the clinical trial examination document, and a clinical trial examination document button; a clinical trial examination document button indication reception part 12 for receiving indication to the clinical trial examination document button; a button-corresponding screen output part 13 for outputting a button-corresponding screen corresponding to the clinical trial examination document button received with the indication when the clinical trial examination document button indication reception part 12 receives the indication; an input reception part 14 for receiving input data to a UI component of the button-corresponding screen; a storage part 15 for storing the input data; a print button indication reception part 16 receiving indication to a print button of the button-corresponding screen; and a clinical trial examination document output part 17 for outputting the clinical trial examination document in an output form corresponding to the print button when the print button indication reception part 16 receives the indication.
